1. Pre-Application Form

• Once contracted, you can complete a one page pre application that can be mailed or faxed.
• Click here to download and print the pre-application form

2. E-Z App Online Submission

• You comple the EZ-Application on-line
• The application is submitted electronically to West Coast Life
• West Coast Life will assign a policy number
• The application is completed by a TeleLife trained insurance interviewer
• The Interviewer will schedule the paramed
• Case Status will be provided by BSI only after the application is completed
• Policies will be mailed to BSI along with any Delivery Requirments
• Your Client returns any and all Delivery Requirements
• BSI Delivers the policy
